Will hip replacement come up in the medical exam?

Will be 58 this year. Considering a career change.
The right hip has been replaced due to an injury.
No loss of sensation. Somewhat weak but improving.
I looked over the medical questions and though it does not ask specifically about this am I required to disclose?

If I disclose would that be grounds for
disqualification? Is it harder due to insurance to
get a job?
